Please complete the following forms and submit to ORUEF. These forms may be completed and submitted online, or they may be printed and mailed/emailed to the ORUEF offices.
Required Forms:
- Annual report (Form A)
- Student Verification Form (complete one for each student) (Form C)
- HSA Roster (Form D)
- Induction Roster (Form E)
Additional Forms (not required):
- Faculty Nomination Form
- Theme Project Planning Form
- Theme Project Final Report Sheet
